چکیده

An algorithm to estimate the transient ST segment level and to construct the ST segment-level functions from 24-hour ambulatory ECG records is presented. The algorithm was developed and tested using the Long-Term ST Database (LTST DB). Initially, the average heart beats are constructed from normal and non-noisy heart beats of the records of the database in a 16-second neighborhood of each heart beat. Then positions of the isoelectric reference point and the J point are located in each average heart beat. The ST segment-level functions are derived for each ECG lead as a difference in the amplitudes at the point of measurement of the ST segment level (80 ms after the J point) and at the isoelectric reference point. The aggregate average error between the amplitudes of the samples of the ST segment-level functions for all 86 records of the LTST DB, of the total duration of 1991:50:49 [h:min:sec], constructed automatically and those constructed using manually determined positions of the isoelectric reference points and J points was only 0.69 μV (st. dev. 8.89 μV). The ST segment-level functions derived contained no significant artifacts.

The Long–Term ST Database: A Research Resource for Algorithm Development and Physiologic Studies of Transient Myocardial Ischemia

We present the Long Term ST Database, a collection of eighty 24-hour two and three lead ECG records from ambulatory subjects with transient ST segment abnormalities. The database provides a comprehensive standard research resource for quantitatively assessing the performance of automated detectors of transient ischemia, and for supporting basic research into the mechanisms and dynamics of trans...

ST segment depression criteria and the prevalence of silent cardiac ischemia in hypertensives.

The reported prevalence of silent cardiac ischemia as assessed by ambulatory electrocardiographic recording varies widely. The influence of the stringency of the analysis criteria has never been reported. We performed 24-hour, 12-lead ambulatory electrocardiographic recording in patients with hypertension but without proven coronary artery disease. The recordings were analyzed according to stri...

Automatic classification of long-term ambulatory ECG records according to type of ischemic heart disease

BACKGROUND Elevated transient ischemic ST segment episodes in the ambulatory electrocardiographic (AECG) records appear generally in patients with transmural ischemia (e. g. Prinzmetal's angina) while depressed ischemic episodes appear in patients with subendocardial ischemia (e. g. unstable or stable angina). Huge amount of AECG data necessitates automatic methods for analysis. We present an a...

Characteristics of episodes of ST elevation or ST depression during ambulatory monitoring in patients subsequently undergoing coronary angiography.

Transient ischaemic ST segment changes were studied in 296 consecutive patients with coronary heart disease attending hospital for coronary arteriography. They underwent two channel, frequency modulated ambulatory monitoring for 24 hours. During this time 221 episodes of transient ST elevation (n = 56) or ST depression (n = 165) with a horizontal deviation of at least 1 mm lasting at least 1 mi...
